| Team Entry forms are required by the Program
Office for every team. Please hand in the form to
the Program Manager running the entry meeting,
or to the Program Office. |
Player Registration

| Every team is required to have their Player
Registration form handed in prior to their second
league game. Failure to do so could lead to a
forfeited game. All participants must sign the
player registration list PRIOR to him/her playing
their first game. |
Players' Pool

| Players' Pool form is for those who wish to sign
up as an individual. When completed, please
drop your form off at the Program Office, where
it will be placed in a binder that is available to
every captain. Please note that the Program
Office will not place an individual on a team. |
Playoff Eligibility

| Playoff Eligibility forms are due before your first
playoff game. Please refer to the Program Office
for specific playoff eligibility requirements for
your sport. |
Official's Application

| Interested in earning some extra money? The
intramural office is always looking for keen
student leaders to officiate in our intramural
program. Please fill out this form and hand it
back in at the Program Office. |
Intramural T
(.pdf file)
| The Intramural T is presented to a student in
his/her graduating year. The "T" shall be awarded
based on an individual's contributions to the
Intramural Program, as a player,
coach/manager, Official, Administrator, or
Committee/Council member. Nomination Forms
are due to the Program Office by the end of
February. |
Player of the Week

| The Intramural Player of the Week forms are due
by Monday, for the previous week's intramural
action. |
Referee of the Month

| The Referee of the Month nomination form are
